How do we work in supporting People

Cornerstones has supported living services in Trowbridge and Devizes, enabling people with learning disabilities to have the independence, opportunities and life chances that we all expect.

At Cornerstones we support individuals to live in a house in the local community as tenants. They pay rent to the landlord or the owner of the house and have a set of keys to enter and leave the house as they wish.

The tenant has an agreement in place which covers their care support and has a nominated key worker who will be the primary support worker.

The house is the tenant’s home. They get to make the decisions and choose what happens in their own home. This includes the food they eat, when they get up and when they go to bed, what they buy, the activities they do and who comes to visit them in their own home.

The support we provide is dependant on the needs of the tenant. We respect the needs and wishes of the tenant whilst ensuring they remain healthy and safe. The role of the support worker is to help the tenant make decisions and live independently.

Planning Support

Cornerstones has a lot of experience in dealing with people with challenging behavior.  This particular group is often the most marginalised within an already marginalised group, but we have found with a completely individual approach to planning support lives can be be changed.

Employment, Training & Education

It is integral part of the supporting people programme to work with tenants to support them in accessing work placements and training programmes which are chosen and developed by themselves. Some of the areas we are constantly involved in is working with local job coaches to access and provide job opportunities to those tenants who wish to seek employment thus increase their community involvment.

Training is accessed through local college agencies and we work individually with tenants to ensure training packages are personal choices. We currently run skills development training in gardening and maintenance  with a personal future goal of one tenant to use skills developed to access paid employment. Within the support services we provide, we offer information sessions regularly on areas like: Fire Safety, Support Planning, Person Centre Planning and many more. Some aspects of the information sessions are being developed by the tenants allowing the opportunity of tenant led sessions.

We are working with one tenant on developing a session on healthy eating and gentle excercise, it has been suggested by the tenant to run a monthly health club which she said would be "nice and helpful" in her own goal of living a healthier lifestyle.

Person Centred Planning

 In person centred planning the process, as well as the product, is owned and controlled by the person (and sometimes their closest family and friends). As there are no prescribed forms, tick boxes or checklists, the resulting plan of support is totally individual. It creates a comprehensive portrait of who the person is and what they want to do with their life and brings together all of the people who are important to the person including family, friends, neighbours, support workers and other professionals involved in their lives.
